Reconstructed after the fact is how a handover pack goes wrong
Most handover packs are written the week after the job ended. That is why most of them are wrong.
It gets built from memory, chased supplier by supplier, and finished the week somebody finally asks for it — usually the week a client moves in and something breaks. Which is the worst possible week to discover the manual is wrong.
The fix is not more effort at the end. On the day, every fact in the pack is easy: what went in, which certificate came with it, what changed from the drawing. Six weeks later every one of them is an archaeology job.

Forty-seven pages for a themed attraction — a whole pack, not a summary of one. Document control and a closed-out comment sheet; a page determining which statutory regime applies and which does not, with the reason; commissioning records including the test that failed; the inspection regime with a failure route for every line; torque, lubrication and obsolescence schedules. Then the half that makes a real handover pack heavy and that most samples leave out: six as-built drawing sheets, the electrical certificate and test results, the safety-circuit validation, the LOLER report, safety data sheets, warranties, coating data and the signage plate — bound in, not referenced.
